Class IV medication Rx Refill
Ambien (zolpidem), Lunesta (eszopiclone), tramadol and all benzos including Restoril (tamazepam)
Service Description
Class IV medications can have a maximum of 5 refills (a total of 6 months). This service type is for only one Class IV medication. If more than one Class IV medication is needed at this time, select the service titled "Add-on medication Rx" for each additional Class IV refill request.
Cancellation Policy
This service is only for patients with established prescriptions and dosages. Prescriptions are not be automatically renewed, even if pharmacy sends an automated requested. A telemed or in-person appointment is needed for any dosage changes, lost or stolen prescriptions or new medications.
