Service Description
For patients in their first 18 months of treatment with Dr. Avery or for patients who need more than just a renewal of stable prescription (for example, lost or stolen medications, new diagnosis, any adjustments to current prescription)
Cancellation Policy
This service is only for those with stable orders, not requiring any modification or adjustments. Within 24 hours of booking and completing the necessary form, you will receive an email informing you that your orders are in place and the date of order expiration. If you require changes to your current order, you must book the other Recertification appointment.
